Team Registration

1) Fill out the Team Roster and Team Waiver Forms below.  Final team roster must be submitted to PBL by end of the 3rd game.  No changes will be allowed after May 27, 2012.  All players require an Individual Waiver Form filled up.

2) An email will be sent to your email address to confirm that the registration has been received.  Please note that registration is not final until team roster has been approved and payment has been received by PBL.

3) PBL will review team roster.  Once approved, an email will be sent to your email address with the Team Roster and Waiver Forms.

4) Complete and mail the Team Roster and Team Waiver Forms with your check payment (payable to PBL of Glendale Heights) to:

5) Once your Team Roster, Waiver Forms and payment are received, an email will be sent to your email address confirming your registration.

Team Fee Incentives:
$1050 per team if full payment is received on or before May 6
$1100 per team if full payment is received between May 7 through May 13
$1150 per team if full payment is received between May 14 through May 20
